Description





Essential Accountabilities:



Administration



+ Assist/empower individuals to identify and achieve recovery goals of employment, education, and community integration.


+ Develop and implement classes, groups and activities within the in PROS (Personalized Recovery Oriented Services) categories:


+ Complete documentation and follow up for assigned caseload


+ Responsible for maintaining/up-keeping all records for their assigned caseload.


+ Coordination of treatment/recovery team meeting(s) for individuals within the program.


+ Communication with assigned prescribers for their caseload.


+ Assist PROS clients in all aspects of their recovery and treatment.


+ Complete referral process for individuals as needed.


+ Provide individual and group rehabilitation/recovery and guidance to the PROS community members.


+ Maintain a caseload of individuals for the purpose of engagement, assessment, and rehabilitation planning and goal setting/acquisition.


+ Provide services in all categories such to facilitate the recovery of individuals diagnosed with mental health conditions.


+ Provide and initiate job development, job placement and community outreach as needed.





Operational





+ Assist/empower individuals to identify and achieve recovery goals of employment, education and community integration.


+ Participate in IPS (Individual Placement and Support) and IRGA team meetings


+ Attend trainings and conferences as needed.


+ Provide coverage, as needed.


+ Participate in regularly scheduled team meetings.


+ Receive supervision by and report to the Director/Supervisor.


+ Function as a vital part of an interdisciplinary team environment.


+ Responsible to report safety issues or concerns that have been brought to their attention.


+ Incident reporting (when applicable).







Qualifications






Qualifications:Minimum of Masters Degree in Social work, MH counseling or Rehab. Experience working with individuals with mental health conditions both individually and in group settings.
